| REFERRAL PROCESS 1) Please call Bernard Raiche, (301) 740-8444 x 222. He will conduct a brief phone consult to determine if PLAN would be an appropriate service for your loved one. If so, then he will schedule an in-office consultation and you will be sent a client information form. Click here to view Client Information Form. 2) Complete the form to the best of your ability and bring it with you to the office consult. 3) In the consult, we will discuss your loved one's clinical needs, the family needs, PLAN policies, procedures, and fees. You will then establish a PLAN contract which will address fees, policies, and initial clinical case management needs. 4) Services for your loved one will be begin as soon as he/she is assigned to a clinical case manager.
