Fees for Services as of 9/2/08

Case Management Services

Hourly rate $80

Charges based on the actual time spent providing services and includes face-to-face sessions, all phone contacts & paperwork.. Time is recorded in quarter hour segments.

When services are provided outside of the PLAN office, there is a charge for travel time and mileage.

Travel time is the same hourly service rate and the mileage rate is the same as that used by the Federal government.

Checking Transactions

Escrow Services are billed monthly and are based on the number of transactions during the month, according to the following schedule:

*  In a month where there are no transactions, Escrow accounts are charged a maintenance fee of $5

Credit Card Transactions

There is a charge for bills which are paid for clients using a PLAN business credit card. Those charges are based on the number of transactions during the month, according to the following schedule:

Trust Management Services

First Year

PLAN Membership                               $ 130

Trust Application Fee                            $ 500

Personal Plan of Care*                          $ 500

Annual Trust Administration Fee            1.5% of Trust Corpus (Maximum fee $7,500)

Investment Management Fee                 0.75% of Trust Corpus

 

Subsequent Years

PLAN Membership                               $ 130 Annual Fee

Personal Plan of Care Review*              $ 160

Annual Spending Plan Review*              $  80

Trust Administration Fee                        1.5% of Trust Corpus (Maximum fee $7,500)     

Investment Management Fee                 0.75% of Trust Corpus

Tax Preparation                                    $ 300  – Depending on complexity
